Why do meniscus tears not heal on their own?

They don’t have a blood supply--except at the edges--so in select tears at the periphery they can heal, but most are out of the region with healing potential. The fluid in the knee also impedes scar formation--and thus lessens healing of repairs away from a blood supply.

How you tear your ACL? Is surgery always required?

A blow to the knee, twisting, planting to change direction, and other forces can exceed the energy absorption ability of this rope like ligament and it tears. Many people can live a normal life without an ACL! Physical therapy, bracing, and life style modification to avoid activities that adversely stress the knee can allow a nearly normal life and still maintain an active healthy lifestyle.

 How are loose bodies removed from the knee? How did they get there in the first place, and why do they cause so much pain?

Pieces can be knocked off the covering over the bone (hyaline cartilage) in many ways. These are then fed by the knee fluid and may grow. They then move, get pinched in the joint and cause mechanical damage, or get stuck in areas that are sensitive--like a kernel of popcorn. Surgical flushing out, or arthroscopically grasping and plucking them out of the joint are the most common way of getting rid of these damaging “joint mice”.
